All SASMI benefits are subject to the terms, conditions and limitations of the Amended and Restated SASMI Trust Agreement and the Rules
and Regulations adopted by the Trustees ("SASMI Plan Documents") and the SASMI Trustees reserve the right to amend the SASMI Plan Documents
at any time. My eligibility for a benefit and my benefit amount will be determined in accordance with the provisions of the SASMI Plan
Documents and taking into consideration my specific facts at the time of my application for benefits.
Information obtained or accessed via the SASMI Participant Portal is solely for the purpose of providing basic information on the benefits
provided by SASMI. The SASMI Trustees strongly recommend that participants refrain from relying exclusively on the SASMI Participant Portal
as a guarantee of a benefit or benefit amount.
If I become aware of the loss, theft or unauthorized use of my Username and Password, I will notify SASMI by phone or in writing as soon
as possible. Any transaction(s) conducted via the SASMI Participant Portal using the Username and Password assigned to me will be deemed
to have been authorized by me, unless I have previously notified SASMI, in a timely manner, of the loss, theft, or unauthorized use of
these personal identifiers.
I will not use the SASMI Participant Portal or any information which may be accessed through SASMI Participant Portal for any
purpose that is unlawful.
I acknowledge that some, but not all of the data maintained by SASMI is covered by the Health Insurance Portability and Accessibility
Act (HIPAA). In connection with data covered by HIPAA the SASMI Trustees have adopted a Notice of Privacy Practices that describes my
rights with respect to my personal information and how the SASMI Trustees will use, disclose and protect such information.
